Counsel Concierge Nursing offers private, professional nursing oversight for independent adults who prioritize consistency and proactive care. We support aging in place while providing families with assurances of clinical expertise and professionalism.
We operate on a private-pay, retainer-based model, allowing care to be tailored to each client’s unique circumstances without the constraints of traditional home health or insurance-based services.
Situational clinical support outside of standard service arrangements is offered on an individual basis following consultation.
Monthly Retainer Services
Following an initial consultation, a retainer is structured around the specific oversight, coordination, and clinical support your situation requires. Listed below are the full scope of services available.

Ongoing clinical monitoring. In-home visits to assess health status, observe changes in condition, and establish a consistent clinical baseline over time.

Provider communication & follow-through. Direct communication with primary care and specialist teams to ensure recommendations are understood, followed, and coordinated across providers.

Discharge & recovery support. Structured oversight during the high-risk post-discharge & post-surgical window. Medication review, follow-up coordination, and early identification of complications.

Clinical monitoring and management of chronic disease. Living with chronic conditions requires requires consistent, knowledgeable oversight. Whether you're managing a single condition or navigating the complexity of several at once, we provide the clinical expertise and personal attention that helps you stay ahead of your health.

Thorough teaching and instruction giving our clients the tools and comprehension they need to become active participants in their care.

Comprehensive reconciliation of all medication, including OTC and supplements, with attention to interactions, high-risk agents, and compliance.

Appointment preparation, accompaniment, and follow-up. Pre-visit preparation to maximize clinical encounters, in-person accompaniment as appropriate, and post-visit follow-up with the family.

Structured updates & reporting. Scheduled family updates that translate clinical observations into clear, actionable information. Client& family access to Counsel Portal.

Proactive risk identification. Anticipating problems before they surface: fall risk, functional decline, polypharmacy, etc. Addressing them with clinical precision to prevent crisis.

ADL assistance and company. While providing professional, supportive nursing services, we assist with ADLs and provide several opportunities for companionship (walks, errands, meals).
Situational Clinical Support
In certain circumstances, we provide case-specific, clinical involvement outside of standard service arrangements. When appropriate, this may include:

appointments
transitional periods, i.e., hospital discharge and surgical recovery.
Availability is determined on an individual basis following consultation. Situational services begin at $100/hr. Please request a consultation if you are interested in situational clinical support.

Counsel operates differently. Services are private-pay, ongoing, and clinical in nature, focused on oversight, coordination, and proactive management rather than task completion or episodic skilled care.

Geriatric care managers are typically social workers or nurses who assess needs and arrange services. Counsel provides direct, ongoing clinical nursing oversight — not a one-time assessment or a referral to other services. The relationship is continuous and clinically active.

Most Counsel clients are independent adults managing one or more chronic conditions, often with adult children or family members who are geographically distant or professionally occupied. Families typically come to Counsel after a hospitalization, a concerning diagnosis, or the recognition that navigating the healthcare system on their own is no longer sustainable. We work best with proactive families who value time and peace of mind.

Every consultation request begins with a complimentary discovery call to assess appropriateness. The initial consultation is a focused clinical assessment and family meeting conducted in the home to assess the client's current health status, health history, medications, functional independence, and family goals. It concludes with a written care plan and recommended retainer structure. There is no obligation to proceed with our services at the conclusion of the consultation process.
